school age program enrollment

Students with a suspected or identified need:

Referrals for students with suspected or identified areas of need must begin with the local school district (the school district in which the student lives). Students must meet certain qualifications to receive specialized services. The local school district will schedule an evaluation called an Evaluation Team Report if indicated.

Students qualify for services based on an Evaluation Team Report (ETR) developed through standardized norm-referenced testing, criterion-referenced/curriculum based evaluation, structured observations, parent interview and record review. When the Evaluation Team Report (ETR) is complete a meeting is held between the school district, parent/guardian, specialists if indicated by the ETR, a representative from Hopewell School if indicated by the ETR and anyone else requested by the parent.

An Individual Education Plan (IEP) is developed for students who qualify for special education services by a team. Members of the team include the student's parents, teacher, school district special education coordinator and may also include the school psychologist, specialists, parent advocate, and anyone else as requested by the parent. The Individual Education Plan includes the student's strengths and needs, family and student preferences, goals and objectives and services to address the student's needs.
