In situations where an individual is at risk for substantial harm, Service and Support Coordinator's (SSC's) take action to alleviate that immediate risk. The Service and Support Administration Office provide support 24 hours per day, 7 days per week. SSA staff work with the individual and/or family to access or link with appropriate providers or resources to address financial, medical, educational, habilitation, housing, placement, therapy, behavior support and other such needs.

Contact to Report a Major Unusual Incident (MUI)

The phone number to report a MUI immediately upon its identification or notification is 740-502-1768. When calling, the caller may be asked his/her name, and a phone number where the caller can be reached; name(s) of the victim(s), name(s) of the primary person(s) involved, witnesses, if any; brief description of the incident(s) and the date/time of occurrence, and; has the necessary action been taken to ensure that the health and safety of the victim(s) regarding the incident being reported.

All written MUI reports are to be provided to Coshocton County Board of Mental Retardation and Developmental Disabilities. Reports can be faxed or hand delivered to Coshocton County Board of Mental Retardation and Developmental Disabilities Office of Service & Support 23720 Airport Road Coshocton, Ohio 43812. Written reports are required by 3:00 p.m. the next working day following the identification or notification of the incident (MUI) report.
